HOW TO REGISTER

How to register: Text

We have an open list and welcome requests for registration from patients no matter their location. If you work in Cricklewood but live elsewhere and will find it easier to see a doctor during our opening hours you are welcome to register as a patient, however, we are only able to perform home visits for those patients inside of our practice area.

We are located at 7 Oaklands Road, Cricklewood, NW2 6DJ. This is behind the Post Office on Cricklewood Broadway. If you have any trouble reaching the practice, please do not hesitate to contact us on 0203 826 8084 and we will be happy to help.

If you are coming to the practice, to save time you can also download a registration form
by clicking here
This form is the NHS standard GMS 1 form that we are obliged to use and which will allow us to locate your previous medical history. In addition, please can you complete our practice registration form by clicking here. This provides us with additional information (not requested on the GMS 1 form) that is essential to your care. We accept these forms via email but only if proof of identification is also attached. We will also accept if the completed form is handed to us in person with proof of identification.

If you enter an email address, we will assume implied consent to pass it on to our patient participation group. If you do not want us to pass your details on, please let us know.

You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.
